Instructions for completion: This Business Associate Agreement includes three parts: (1) Part I (“Agreement and Covered Entity Information”) shall be completed by Covered Entity (defined below); (2) Part II (“Covered Entity Execution”) should be signed by a duly authorized representative of Covered Entity; and (3) Part III (“Agreement Terms and Conditions”) state the obligations of ConnectYourCare (“CYC”) and Covered Entity with regard the use and disclosure of health information that is protected by federal law as defined below (hereinafter “Protected Health Information”).
