This Amendment to Contract No.: DMS-15/16-003 (the “Contract”) is by and between the State of Florida, Department of Management Services (the “Department”), and E4 Health, Inc. (“Assignor”), and E4, LLC (“Contractor” or “Assignee”) a wholly-owned subsidiary of New Directions Behavioral Health, LLC.
