This Addendum to the Hospital Participation Agreement (the “Addendum”) is between the hospital identified above (“Hospital”) and the American College of Surgeons (“ACS”) related to the ACS Metabolic and Bariatric Surgery Center Accreditation and Quality Improvement Program (“MBSAQIP”). The parties agree to amend the Hospital Participation Agreement (“Agreement”), the Business Associate and Data Use Agreement (“BA/DUA”), and all other written agreements between Hospital and ACS as follows:
