This Agreement is between the State Treasurer of Ohio (“Treasurer”), 30 East Broad Street, 9th Floor, Columbus, Ohio 43215-3461 and _ (“Financial Institution”) (ADDRESS), who will collectively be referred to as “Parties”. The Agreement is authorized by the provisions of Ohio Revised Code (“ORC”) § 113.051. The term of this Agreement is July 2, 2012 through July 6, 2014 and may be extended by written agreement of the Parties. This Agreement sets forth the custody services to be provided by the Custodian for the accounts of the Beneficial Owner(s) listed in Exhibit 1.
