This Acknowledgement of Proprietary Rights and Non-Disclosure and Agreement (“Agreement”) is entered into between the American Heart Association, Inc., a New York not-for-profit corporation, having its principal offices at 7272 Greenville Avenue, Dallas, Texas 75231-4596 ("AHA") and enter Institution name (“Institution”) on behalf of its faculty member, enter Investigator name (“Investigator”).
