This Data Recipient Agreement (“Agreement”) with respect to a De-Identified Data Set is entered into by and between University of Massachusetts Medical School (hereinafter referred to as “Data Provider”), a public institution of higher education within the Commonwealth of Massachusetts, 55 Lake Avenue North Worcester MA 01655 and [Recipient] (hereinafter referred to as “Data Recipient”).
