INTRODUCTION - By signing this General Terms Agreement ("GTA" or "Agreement") I am entering into this Agreement with EPA Sports Training ("EPA Sports") on behalf of myself as Primary Member and any current or future named Additional Members, or as a non-member on behalf of and as purchaser, payor and guarantor for a Primary Member and any named Additional Members (a "Third Party Purchaser"), whether such member configuration remains the same for all Members on the Membership, or is subsequently changed, in order to obtain the benefits of a membership ("Membership") that affords a Member general or limited access to one or more of EPA Sports' health and fitness centers ("Centers") and/or allows a Member to schedule and/or enroll in and have access to certain services offered by EPA Sports from time to time.
