This Resident Employment Agreement (“Agreement”) by and between Great Plains Medical Foundation, LLC d/b/a Great Plains Family Medicine Residency Program, an Oklahoma limited liability company (the “Foundation”), and RESIDENT (the “Resident”) is made and entered into effective July 1, 2015 (“Effective Date”).
