This Agreement is entered into between the State of Minnesota, acting through its Board of Trustees of the Minnesota State Colleges and Universities, on behalf of [insert college/university name] (hereinafter “College/University”), and ST LUKES HOSPITAL OF DULUTH, 915 E 1ST ST, DULUTH, Minnesota 55805 (St. Lukes), on behalf of its affiliate healthcare organizations, Lakeview Memorial Hospital, Two Harbors, Minnesota, and Pavilion Surgery Center, 920 East First Street. Suite 101, Duluth, Minnesota 55805 and includes all entities which become affiliated with St. Lukes following the effective date of this Agreement. St. Lukes shall notify College/University in writing of any new or subsequently acquired St. Lukes entity to be covered under this Agreement and the effective date of affiliation. (hereinafter referred to individually and collectively as “Facility”).
