Mind & Body Medical Clinic is a fee-for-service and direct pay specialty care clinic (DPC), which delivers primary care services through its PROVIDERS: physician, Dr. Dustin Rosenhamer, MD (Physician) and Kelli Leek APRN-CNS, (advanced practice nurse) at 2216 NW 164th St, Suite A, Edmond, OK 73013. In exchange for certain fees, the CLINIC, agrees to provide You with the Services described in this Agreement on the terms and conditions contained in this Agreement.
