This Agreement for Emergency Medical Service User Fee Billing Services (“Agreement”) is made and entered into between the City of Kenosha, Wisconsin, a municipal corporation, with offices located at 625 52nd Street, Kenosha, Wisconsin 53140, hereinafter referred to as the “Client”, and Andres Medical Billing, Ltd., a foreign corporation, with its principal offices located at 3223 North Wilke Road, Arlington Heights, IL 60004, hereinafter referred to as the “Service Provider”. This Agreement shall be effective upon approval and execution by the Parties. The effective date shall be the date of last execution of this Agreement (“Effective Date”).
