It also contains information about privacy protections and client rights with regard to the use and disclosure of your Protected Health Information (PHI) used for the purpose of treatment and payment. Although this document is long and sometimes complex, it is very important that you read it carefully before beginning your first session. Your therapist can discuss any questions you have about the procedures explained in this document. When you sign this document, it will also represent an agreement between us.
