Each student and his or her parent/guardian must read and sign this Agreement to Participate along with the IHSA Sports Medicine Acknowledgment & Consent Form (Concussion Information and Consent) each year before being allowed to participate in interscholastic athletics. Parents/Guardians will give consent through our online registration system. Students will sign the concussion consent as they log into their portal. Students interested in participating in interscholastic sports should sign below and turn in this form along with the completed IHSA/IESA Pre-Participation Form.
