THIS AGREEMENT IS MADE and entered into by and between CITY OF HUDSON, with offices located at 505 Third Street, Hudson, WI 54016, hereinafter referred to as the “CLIENT”, and EMS MEDICAL BILLING ASSOCIATES, LLC, a Wisconsin Limited Liability Company, with offices located at 9401 W. Brown Deer Road, Suite 101, Milwaukee, Wisconsin 53224, hereinafter referred to as the “SERVICE PROVIDER”
