THIS PROFESSIONAL SERVICES AGREEMENT (“Agreement”) is made and entered into this day of 202 , (the “Effective Date”) by and between the Altadena Library District, a California Public Utilities District (“District”), and (“Consultant”). District and Consultant may herein be referred to individually as a “Party” and collectively as the “Parties.”
