Authorization Agreement I hereby authorize Little River Band of Ottawa Indians to initiate automatic deposits to my account at the financial institution named below. I also authorize Little River Band of Ottawa Indians to make withdrawals from this account in the event that a credit entry is made in error. I also authorize Little River Band of Ottawa Indians to cancel automatic deposits if the account information is incorrect or member information on this form does not match the information on file with the Enrollment Department. Further, I agree not to hold Little River Band of Ottawa Indians responsible for any delay or loss of funds due to incorrect or incomplete information supplied by me or by my financial institution or due to an error on the part of my financial institution in depositing funds to my account. This agreement will remain in effect as long as member information and account information on file is correct or Little River Band of Ottawa Indians receives a written notic
