is made as of the 25th day of November, 2013 (the “Effective Date”) by and among Jersey Shore University Medical Center, a Division of Meridian Hospitals Corporation (“JSUMC”) and the practice identified on the signature page hereof (the “Practice”) and physician(s) (“Physician” or “Participating Physician”) identified on Exhibit A. JSUMC, the Practice and the Participating Physician are the “Parties,” and each is a “Party.”
