This Business Associate Agreement (“Agreement”) dated , 2022 (the “Effective Date”), is entered into by and between THE HOSPITAL AUTHORITY OF VALDOSTA AND LOWNDES COUNTY, GEORGIA, d/b/a South Georgia Medical Center, a body corporate and politic, created pursuant to the Hospital Authority Law, hereinafter referred to as “Authority” and hereinafter referred to as “ASSOCIATE”, for the purposes of complying with the privacy and security regulations issued by the United States Department of Health and Human Services under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the security provisions of the American Recovery and Reinvestment Act of 2009 (“ARRA”). Authority and ASSOCIATE may be individually referred to as “Party”, and collectively referred to as the “Parties.”
