This MASTER SERVICES AGREEMENT (the “MSA” or “Agreement”) is made by MARYLAND BROADBAND COOPERATIVE, INC., a Maryland consumer cooperative corporation with its principal office at 2129A Northwood Drive, Salisbury, MD 21801 (“MdBC”) and with its principal office at , (“CUSTOMER”), effective as of the date of last execution (the “Effective Date”). MdBC and CUSTOMER agree that the following Terms and Conditions apply to the provision and use of the products and services referenced herein and in any Supplements and Service Orders. Either MdBC or CUSTOMER may be referred to herein individually as a “Party’” or, collectively, as the “Parties.”
