The agreement must be signed and returned to The One Project. If the Participant (as shown below) is not 18 years of age or older on the date of execution of this Agreement, this Agreement also must be signed and notarized by the parent or guardian. Failure to accept and/or abide by the terms and conditions of this Agreement as provided may result in the Participants’ inability to participate in the mission trip.
