This Fourth Amendment to Agreement (“Fourth Amendment”) is made by the County of Alameda (“County”) and Alameda Health Consortium, (“Contractor”) with respect to that certain agreement, Procurement Contract No. 7356, entered by them on May 1, 2012 and that certain First Amendment, Second Amendment, and Third Amendment to Agreement, (collectively referred to herein interchangeably as the “Contract” or “Agreement”) pursuant to which Contractor provides services as part of a larger Behavioral Health Care Services (BHCS) effort to invest in the development of increased and enhanced behavioral health screening, assessment, and treatment capacity within the primary care clinics participating in the Health Program of Alameda County (HealthPAC).
