WHEREAS, the purpose of this AFFILIATION AGREEMENT is to guide and direct the parties respecting their affiliation, working arrangements and agreements in furtherance thereof to provide high-quality learning experiences for clinical students in the Educational Institution’s Programs and/or Departments listed in Exhibit B, hereinafter referred to as “Program”. This AFFILIATION AGREEMENT (the “AGREEMENT”) is made between TEXAS WOMAN’S UNIVERSITY SYSTEM, a public institution located in Texas, hereinafter referred to as “TWU,” and the [EDUCATIONAL INSTITUTION NAME HERE], an entity physically located in [STATE], hereinafter referred to as “Educational Institution.” TWU and Educational Institution shall be collectively referred to hereinafter as the “Parties.”
