FOURTH AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO ANDOur Common Ground, Inc. THIS FOURTH AMENDMENT TO THE AGREEMENT, entered into this day of , 20 , by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Our Common Ground, Inc., hereinafter called "Contractor"; W I T N E S S E T H: WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof; WHEREAS, on June 23, 2009, the parties hereto under Resolution 070228 entered into an agreement, (the “Original Agreement”), for the furnishing of alcohol and drug services by Contractor to County as set forth in that Agreement; and WHEREAS, on January 15, 2010, the parties, under a First Amendment, modified the Original Agreement to decrease the Drug Medi-Cal (DMC) rate, and add funding in the amount of $5,891, for a new maximum obligation of $5,243,115 dollars; and WHEREAS, on April 13, 2010, the
