The undersigned parent(s) or legal guardian(s) of my child (named below) hereby give permission for him/her to participate in any Highland Park Presbyterian Church (HPPC) program or event, including any Nursery, Early Childhood, Elementary, Jam56, Middle School, High School, or other Children or Youth program or event. These programs may include field trips, swimming outings, mission trips, or retreats and can also include transportation to and from such events, whether in the HPPC church vans, private volunteer driven cars, buses leased by HPPC from professional transportation companies, air travel, or other transportation means. I (we) understand that this grant of permission is not specific to one event, but shall include all events (“the events”) in which my child participates under the auspices of Highland Park Presbyterian Church’s Children & Youth Ministry.
