Applicant/Student (hereinafter referred to as “Student”) has had the course(s) fully and completely explained to him/her and agrees to abide by the terms listed in this Enrollment Agreement and by the rules and regulations of Pura Vida Divers (hereinafter referred to as “School”). This agreement consists of two pages in conjunction with the catalog and constitutes the entire agreement between the School and the Student. The Student understands that if the School accepts this agreement, it shall be a binding contract and no further modification or representation except as herein expressed by both parties will be recognized.
