This Participation Agreement is entered into by and between the Board of Trustees of the Pipe Fitters’ Welfare Fund, Local 597 (the “Fund”) and (“Employer”), effective no earlier than January 1, 2022, or if the date of execution by the Employer is later than January 1, 2022, then this Participation Agreement is effective the first day of the month following the date of execution by both parties (“Effective Date”).
