This HIPAA Business Associate Agreement (“BAA”) is by and between the National Association of Boards of Pharmacy (“NABP” or “Business Associate”) and Verified Pharmacy Program Organization and facility/facilities, (“Covered Entity”) (collectively between NABP and Covered Entity referred to as “parties”.) The parties agree that the BAA is effective as of the earlier of the following dates: the Effective Date of the Agreement or upon the date that Protected Health Information is or was acquired, accessed, used or disclosed.
