The following provisions constitute an Electronic Communication Delivery Authorization and Agreement (this “Authorization”) with Farmers & Merchants Bank, Lakeland, Georgia (hereinafter referred to as “Bank”, “we”, “our”, and “us”) concerning the electronic delivery of Communications concerning accounts maintained by the customer who accepts this Authorization (hereinafter referred to as “Customer”, “you”, “yours”, and “depositor”). “Communications” shall mean any communications between you and us and includes but is not limited to periodic statements; disclosure statements about your account such as Electronic Funds Transfer Disclosures, Annual Percentage Yields, Funds Availability, Truth in Lending Act, etc; Privacy Statements; changes and amendment to this and other agreements; and any and all notifications.
