, (Provider) enters into a Provider Agreement with the State of Alaska, Department of Health & Social Services (DHSS) for the purpose of providing Individualized Services for referred clients of the Division of Behavioral Health (DBH), Office of Children’s Services (OCS), the Division of Juvenile Justice (DJJ) and self-referred individuals for the State of Alaska’s Residential Care for Children and Youth (RCCY) Program. By entering into this Provider Agreement, the Provider agrees to maintain, in good standing, the Core Services Grant Agreement required for the provision of services under this Provider Agreement, as well as the following requirements, including all applicable provisions of the following Appendices:
