THIS BUSINESS ASSOCIATE AGREEMENT (“Agreement”) is made and entered into by and between the organization who wishes to access KDQOL-Complete™ (“Covered Entity”) and Medical Education Institute, Inc., a Wisconsin non-stock/non-profit corporation (“Business Associate”) (collectively, the “Parties”).
