The present General Conditions and the Special Conditions and the Schedule of Special Conditions attached hereto related to this Policy, and the application submitted by the Assured, together with the declarations made by the Assured, in addition to the statements made by the Assured during the medical examination, if any, and any Supplementary Assurance or Endorsement attached to this Policy, forms the entire Policy entered by and between the Assured and the Company. All the statements made, however in the absence of fraud, by the Assured in applying and on the application for this Policy shall form the basis of this Policy. This Policy shall be incontestable as from its date of issue, unless proof of fraud or false declaration deliberately and resolutely was submitted. This Policy shall always be subject to the conditions and amendments hereupon endorsed, if any, and to the extent where it is applicable, and which shall be deemed to be an integral part of this Policy.
