This HIPAA Business Associate Agreement (“HIPAA BAA”) is made between Jotform, Inc., (“Jotform”) and West Branch Drug and Alcohol Abuse Commission (“Covered Entity” or “Customer” ) as an agreement to the Jotform Terms of Use (the “Terms of Use”). This HIPAA BAA is effective as of April 9, 2021 (“Effective Date”), which is the date Customer indicated its acceptance of this HIPAA BAA electronically. This HIPAA BAA was electronically signed by Shea Madden, Executive Director on behalf of Customer on the Effective Date.
