I, , understand and agree to comply with all rules and regulations set forth in any Fitzpatrick park waivers and bylaws as well as any posted on park property, websites, or social media. I accept full responsibility for my actions towards other players, park goers, park staff, league officials, and umpires. Should I choose to violate any of these rules or regulations I understand that I will be subject to disciplinary actions including but not limited to: being removed from a game, game forfeiture, and/or a temporary or permanent ban from Fitzpatrick park including any and all tournaments.
