ANTONIO (“Performing Agency”, “Grantee”), who are collectively referred to herein as the "Parties," entered into the following contract for Office-Based Opioid Administrative Services (“OBT Admin”) services (the “Contract”), effective September 12, 2019, and now denominated HHSC Contract No. HHS000561800001 (“Contract” or “Program Attachment”) or, as amended, now desire to further amend the Contract.
