INSURANCE: We currently work with a number of different insurance companies. Please check with us to see if we participate with your insurance. For those companies we work with, you will be responsible for paying any applicable copayment or deductible at the time of service. Claims will be submitted on an assigned basis, and benefits will be paid directly to our office. Please note that, under certain circumstances, your insurance plan will not pay for our services and you will be responsible for payment in full. All fees are ultimately your responsibility. INSURANCE COVERAGE IS NOT A GUARANTEE OF PAYMENT.
