Any disabled employee receiving a Short Term Disability (STD) or Long Term Disability (LTD) benefit under a Sun Life and Health Insurance Company (U.S.) (“Sun Life”) group insurance policy must receive a W-2 form that sets forth taxable and non-taxable disability income, and taxes withheld, if any.
