All students bringing a non-Boulder Creek High School student must complete this form BEFORE they can attend the dance. This form is due by April 24, 2024 at 2:30PM. Incomplete forms will not be accepted. Prom contracts will NOT be accepted after Wednesday, 4/24/24 at 2:30PM. No Exceptions! A BCHS student can only bring ONE GUEST. You will receive an email (to your DVUSD learner email address) from Mrs. Tunnell when your form has been approved or denied.
