PURPOSE: The purpose of this Agreement is to provide the terms, covenants and conditions under which Service Provider will provide services to persons to whom the County refers Service Provider during the COVID-19 public health emergency. In exchange for Service Provider signing this Agreement, the County may refer Service Provider to persons the County believes Service Provider may be able to help during the COVID-19 public health emergency.
