This Amendment is made part of the Administrative Services Agreement for Jointly Administered Arrangements (Agreement) and is effective January 1, 2021. This Amendment supplements and amends the Agreement between Client, Advantek ("TPA") and Anthem Blue Cross Life and Health Insurance Company dba Anthem (“Anthem”). If there are any inconsistencies between the terms of the Agreement or its Schedules and this Amendment, the terms of this Amendment shall govern.
