Please sign and return to Camelot Property Management 37 West 26th Street, New York, New York 10010, Attention: David Goldoff, and include all applicable documents listed in the Alteration Agreement. Only complete packages, which include all certificates of insurance and other required documents, will be reviewed and approved. Please note that incomplete packages will bereturned.
