The Department of State Health Services (“DSHS” or “System Agency”) and The University of Texas Health Science Center at San Antonio, State Agency Number 745 (“Performing Agency”), each a “Party” and collectively the “Parties,” enter into the following agreement for services provided under the Oral Health Improvement Program (“OHIP”) (the “Contract”) pursuant to the provisions of “The Interagency Cooperation Act,” Chapter 771 of the Texas Government Code.
