This Ancillary Service Agreement (“Agreement”) is entered into by and between HealthSpring Life & Health Insurance Company, Inc., Cigna Health and Life Insurance Company and Cigna Health Care of Colorado, Inc. on behalf of themselves and their Affiliates (“Cigna- HealthSpring”) and Western Rehabilitation Health Network, L.C. on behalf of itself and the Ancillary locations listed on the attached Exhibit A (“Facility”). The Agreement shall go into effect on the “Effective Date” specified on the signature page below. Cigna-HealthSpring and Facility are each a “Party” to the Agreement and they may individually or collectively be referred to (as the context may require) as a “Party” or the “Parties”.
