This Patient Safety Organization Services Agreement (“Agreement”), is entered into effective , 20 (the “Effective Date”), by and between Clarity PSO, a division of Clarity Group, Inc., a Delaware for-profit corporation, (“Clarity PSO”), and the Provider, (“Client”), who wishes to participate in the Radiation Oncology Incident Learning System sponsored by the American Society for Radiation Oncology (“ASTRO”) and the American Association of Physicists in Medicine (“AAPM”).
