THIS EDUCATION AFFILIATION AGREEMENT (“Agreement”) is made and entered into by and between the Utah State Office of Education (“USOE”), on behalf of each individual Utah School District included as a signatory and as evidenced by the signature of each School District’s respective agent, (hereinafter referred to individually as “District” and collectively as "Districts") and IHC Health Services, Inc., a Utah non-profit organization ("IHCHS"), on behalf of its operating units, service lines and facilities (hereinafter referred to individually as “Facility” and collectively as “Facilities”), for participation in the Work-based Learning Program (“WBL Program”). The parties agree that the Districts involved in the WBL Program may change from time to time. Districts may be added to or deleted by providing written notice. Such changes shall be noted in writing, signed by the respective District.
