This Voluntary Consent, Waiver, and Agreement for Use of School Bus Transportation (“Agreement”) is made and entered into between Mount Madonna School (the “School”) and the undersigned parent(s) or legal guardian(s) (“Parents”) of (“Student”). The Parents and the School are sometimes referred to individually as “Party” or collectively as the “Parties.” Parents, on behalf of their heirs, executors, administrators, and assigns, and on behalf of the Student, hereby agree to the following terms and conditions:
