This MASTER EDUCATION AFFILIATION AGREEMENT (“Agreement”), effective as of September 1, 2019 (“Effective Date”) is entered by and between JOHN MUIR HEALTH, a California nonprofit public benefit corporation, on behalf of all of its hospitals and locations (“JMH”), and JOHN MUIR HEALTH PHYSICIAN NETWORK, a California nonprofit public benefit corporation, on behalf of all of its primary care and outpatient clinics (“JMHPN”) (collectively “JMH”) on the one hand, and Mt. Diablo Unified School District, a California Public High School District (“SCHOOL”), on the other hand. JMH and SCHOOL are sometimes referred to in this Agreement individually as a “Party” or, collectively, as the “Parties.”
