This Custodial Account Agreement (the “Agreement”) is made between Equity Trust Company (“Custodian”) and each Account Holder who executes Section 2 of this Agreement for the purpose of establishing a Custodial Account (“Account”) to custody all assets deposited into or collected with respect to such Account. Account Holder hereby designates Custodian to serve as custodian of this Account pursuant to the terms set forth below.
