This is to certify that FOR VALUE RECEIVED, , (“Holder”), and LevelBlox, Inc., a public corporation in the State of Delaware, U.S., (the “Company”), subject to the provisions of this Agreement, up to shares of fully paid, validly issued and nonassessable shares of Common Stock of the Company (“Common Stock”), par value $.01 per share, at a price per share equal to Ten Cents ($0.10) (the “Exercise Price”), which exercise may take place at any time or from time to time on or before 5:00 p.m., eastern standard time, on the date that is three (3) years FROM THE DATE HEREOF, or, if such date falls on a Saturday, Sunday, or bank holiday, the next date that is not a Saturday, Sunday, or bank holiday (the “Expiration Date”). Warrant shares are only available for Fifteen Percent (15%) of total shares purchased in Exhibit A Subscription Agreement and requires a subscription of at least Five Hundred Thousand (500,000) Shares, worth Fifty Thousand ($50,000) USD.
