The above-named applicant (hereinafter referred to as “Customer”) hereby authorizes The Mat Source, LLC, a Mississippi limited liability company, and its subsidiaries (hereinafter collectively referred to as “TMS”), and any credit bureau or other investigative agency retained by TMS, to investigate Customer’s creditworthiness and financial responsibility, specifically including, without limitation, express authorization to contact all references listed above. Customer authorizes all creditors, financial institutions and credit agencies to disclose to TMS all requested information regarding Customer’s finances and financial affairs, and a copy of this Commercial Credit Application shall be good and sufficient proof of such authorization. Customer hereby releases and discharges TMS, and its officers, directors, shareholders, employees and agents, from any and all claims, damages or liabilities arising out of, or resulting from, any investigation conducted by TMS, or any credit bureau or
