By signing this form you agree & understand that Badolato Refrigeration LLC will be paid in full under agreed terms. Any past due balance will be charged a monthly interest rate of 1.5% until balance is paid in full. All equipment/parts listed on open invoices remain the property of Badolato Refrigeration LLC until paid in full. Badolato Refrigeration LLC reserves the right at any time to remove equipment listed on open invoices until paid in full. Badolato refrigeration will add bank fees and a $60.00 inconvenience fee to collect on bad debt/returned checks.
